“Fase de retenção e sistemas: o que devemos saber e o que gostaríamos de saber”

A fase de retenção é considerada um ponto crítico para manter uma oclusão boa e estável após um tratamento ortodôntico ativo. No entanto, geralmente faltam evidências neste campo e os estudos de retenção são difíceis de serem realizados em um ambiente clínico típico. É a estabilidade do resultado afetada pela qualidade do resultado e acabamento do tratamento ortodôntico? Por quanto tempo a fase de retenção deve ser realizada? Qual é o prazo que um ortodontista deve ser considerado responsável pelos resultados do tratamento? A duração e os sistemas usados para a retenção devem mudar de acordo com a complexidade do caso, estratégias de tratamento, idade do paciente e fatores individuais? Qual é o papel das mudanças fisiológicas e do envelhecimento da posição dentária em comparação com a recidiva pós-tratamento? Essas são algumas das dúvidas mais comuns que o ortodontista pode ter em relação à fase de contenção. O objetivo desafiador desta apresentação é analisar esses pontos e verificar as possíveis respostas que podem surgir das evidências e da experiência clínica de pacientes tratados e acompanhados, sobre os sistemas, os materiais e a duração do período de retenção nos diferentes diagnósticos de pacientes, plano de tratamento e situações de resultados.

“Retention phase and systems: what we should know and what we would like to know”

The retention phase is considered a critical point in order to keep a good and stable occlusion after an active orthodontic treatment. However, evidence in this field is generally lacking and retention studies are difficult to be performed in a typical clinical setting. Is it the stability of the result affected by the quality of the orthodontic treatment outcome and finishing? For how long the retention phase should be performed? What’s the time frame an orthodontists should be considered responsible for the treatment results? Should the duration and systems used for the retention change accordingly with the case complexity, treatment strategies, patient’s age, and individual factors? What’s the role of the physiologic and ageing dental position changes compared with the post-treatment relapse? These are some of the most common questions that orthodontists may have in relation with the retention phase. The challenging aim of this presentation is to analyze these points and verify the possible answers that can come out from the evidence and from the clinical experience of treated and followed up patients, about the systems, the materials, and the duration of the retention period in the different patient’s diagnostic, treatment plan and results situations.
